She was blacklisted from mainstream medicine, so she practices here — treating locals who can't afford or don't trust hospitals. Her prices are reasonable, her bedside manner is sardonic, and her experimental drugs are genuinely effective. For the Phantom Thieves, this clinic is a lifeline. Takemi's custom medicines heal in the Metaverse in ways standard pharmacy items can't. The more Joker helps with her research (by being a guinea pig), the better the medicine gets.
