Its original purpose was to cure Malenia's scarlet rot by severing the connection between her and the Outer God of Rot. When Miquella was stolen by Mohg, the needle was left incomplete. The Tarnished can finish it and use it in the one place where time stands still — Crumbling Farum Azula — to reverse the Frenzied Flame's hold on their body. The needle does not destroy the Outer God's influence; it excises it, like a surgeon removing a tumor.
