Millicent is one of several 'buds' of Malenia — entities that grew from the scarlet rot bloom that consumed Caelid. She begins the game afflicted and dying, restored by a golden needle that suppresses the rot. Her journey follows Malenia's trail across the Lands Between, driven by a need to understand what she is. Her questline culminates in a choice: help her resist the rot and die human, or let her sisters consume her and bloom into another Goddess of Rot. The former is the 'good' ending — she thanks you, removes the needle, and dies peacefully refusing to become what she was made to be.
