This is THE Death Note — Sidoh's notebook, stolen by Ryuk and dropped into the human world. It passed through Light's hands (first Kira period), then Higuchi's (Yotsuba arc), then back to Light's (second Kira period). It is the primary instrument of mass murder in the series. The notebook's power is absolute within its rules: write a human's name while picturing their face, and they die. Default death is heart attack within 40 seconds. You can specify cause and circumstances of death if written within 6 minutes and 40 seconds. You can control the victim's actions before death. The rules cannot be broken, cannot be circumvented, and do not require the user to be near the victim. Light added two fake rules to this notebook: the 13-day rule ('if you don't write a name within 13 days of your last, you die') and a destruction fake ('if you destroy the notebook, all who have touched it die'). These were written by Ryuk at Light's request to clear Light and Misa of suspicion.

Appearance

A plain black notebook, slightly larger than a standard composition book, with 'Death Note' written in stylized white English text on the cover. The pages are ordinary lined paper. The rules are written inside the front cover in English. It looks like something you'd find in a stationery store — which is part of the horror. The most dangerous weapon in human history looks like a school notebook.
